1) Click on the Forms button in your account's main navigation.

2) Click on the form you'd like to use/share access to.

3) Click on the Use button.

4) From the drop down menu, choose the button text you'd like to use.

5) Copy the HTML code.

6) Paste the HTML code on your website.
