How do I create a payment form? (new design)

For accounts on the new MoonClerk dashboard design (created September 2026 or later).

Written by Amitabh Tewari

This article covers MoonClerk's new dashboard design. If your account still uses the classic design, see the classic version of this article.

The form builder puts everything on one page: your form's Basics and Amount at the top, then optional sections for Payment methods, Add-ons, Confirmation, and Organization. A live Checkout preview sits beside the builder, and you can Save at any point — new forms stay in Draft until you Publish them, so nothing reaches your payers by accident.

A two-minute look at the Modern builder — the whole form on one page, with the live checkout preview beside it.

Instructions

1) Click on the Forms button in your account's main navigation and then click the Create Payment Form button.

2) Basics — give your form a title. It appears at the top of your checkout page, so keep it short and action-oriented. You can also include an optional description.

3) Amount — choose Set Specific Amount(s) to list the amounts payers pick from, or Decided at Checkout to let payers enter their own. Click + Add amount for each amount (with an optional label) and choose whether amounts display as radio buttons or a drop down. Then open the Frequency row and choose One Time, Recurring, or Decided at Checkout (which lets the payer choose whether to pay once or subscribe).

4) Everything below the Optional divider can be skipped. Click a section's row to expand it: Payment methods (what payers can use to pay), Add-ons (coupons, fees, terms of service, theme, availability, and custom fields), Confirmation (what happens after payment), and Organization (tags).

5) Click Save to keep the form as a draft while you work — the Checkout preview on the left shows the page your payers will get, on desktop or mobile.

6) When you're ready, click Publish at the top of the page to make the form live. A form needs a title and at least one amount before it can take payments.

Prefer a different layout? Under Account > Settings you can switch to the Step by step builder (one section at a time) or the Classic editor.
