1) Navigate to the Extensions tab in the MoonClerk dashboard and click on the MoonClerk Pro for Mailchimp extension.
2) In the Form Connections section, click the "...” icon on the form you’d like to remove.
3) Click the “Remove”button.
4) Click the "Remove Connection" warning button to confirm.
