1) Click on “Extensions” in the main navigation of the dashboard.

2) Click on  "Digital Delivery"

3) Click on “Packages”

4) Click on the connected package you’d like to disconnect from a form.

5) Scroll to the Form Connections section and click on the “X” icon of the form whose connection you’d like to remove. 

6) Click on the “Remove” button to confirm.
