Our default form asks for the payer's name, email, credit card number, expiration date, and CV code. You have the option of asking for as much information as you'd like. The fields that we currently have available in our form builder are:

  • Checkboxes

  • Address

  • File Upload

  • Phone Number

  • Date

  • Time

You can add unlimited fields, give them custom titles, write help/instructional text for each one, rearrange their order, and choose whether they are required/optional.
