1) Click on the Forms button in the dashboard main navigation

2) Click on the form you'd like to use.

3) Click on the Use button.

4) Scroll down to find the Wordpress Shortcode section. Copy the shortcode.

5) Use the shortcode with the MoonClerk Wordpress plugin.
